Salary Information:
This position traditionally starts at an annual salary of $37,778.  Salary for this position may be commensurate with education and job experience.

About the Job:

Supervised by the Cardiovascular Health and Diabetes Section Director of the Indiana State Department of Health.  This position serves as the program lead for the Diabetes Program; assists in planning, developing, coordinating, implementing and evaluating evidence-based diabetes and diabetes prevention initiatives, with a primary focus on achieving goals and objectives as required by federal and other funding designed to reduce the burden of diabetes in the state. Additionally, the Diabetes Program Director develops and monitors organzational/partner contracts and grants, and provides technical assistance, including developing or linking people/organizations to resources and coordinating statewide activities.

A Day in the Life:

The essential functions of this role are as follows:

  • Assist IDOH in planning, developing and coordinating and implementing activities, initiatvies and  work plans to meet the state’s federal grants and state objectives on diabetes and prevention of diabetes.
  • Assist the Section Director in writing organziationalcontract and grant deliverables and creating their contracts or grants
  • Provide technical assistance to partners and other external stakeholders on implementing and coordinating activities to meet federal grant goals and objectives.
  • Assist partner organizations with evidence-based treatment and management of diabetes and the prevention of diabetes for patients. 
  • Work with the community health worker partners and other stakeholders to promote the use of and sustainability of community health workers in the areas of diabetes and prediabetes, and to implement activities surrounding community health workers within the Section’s grant funding.
  • Network with various community groups to link them to community health centers and other health systems, including nursing schools, hospital systems, WISEWOMAN program, pharmacies, parish nursing, etc.
  • Work with the Office of Primary Care (OPC) and Office of Rural Health staff on the Indiana Primary Care Learning Collaborative (INPCLC) project with federally qualified health centers (FQHCs) community health centers (CHCs) and rural health centers (RHCs) and IDOH WISEWOMAN staff in providing technical assistance to FQHCs/CHCs/RHCs on diabetes treatment and management and the prevention of diabetes; and evidence-baed programs.
  • Assist OPC and CVHD Section staff on setting up conference calls, educational webinars and trainings for health system grantees and partners.
  • Work with IDOH OPC staff, the CVHD Section Epidemiologists, and Evaluators to identify data sources, subpopulations, and communities in Indiana in order to implement activities and interventions, and to measure performance.
  • Provide trainings and presentations as needed to implement grant strategies and activities.

What You'll Need for Success:

  • Bachelor’s Degree in any of the following:  public health or community health, nursing, dietetics, pharmacy, health education, or similar degree.  Two years relevant experience needed – in public or community health settings preferred.
  • Ability to effectively establish effective relationships with external partners.
  • Able to keep abreast of latest research, guidelines, and recommendations for diabetes and prediabetes.
  • Writing skills for the creation of reports.
  • Experience with project coordination in an interdisciplinary/team environment.
  • Strong written and verbal communication skills.
  • Computer knowledge and skill in Microsoft Office, Word, PowerPoint, and Excel.
  • Ability to establish and maintain effective working relationships with federal, state, local community and/or health organizations and other stakeholders.
