*Hire Salary: The starting salary for the Family Case Manager position is $35,776 annually. Upon successful completion of the cohort training, the salary will be increased to $40,092 annually. Cohort training lasts approximately 12 weeks and is subject to change contingent upon successful completion of classroom and learning activities

About the Job:

The Child Services Collaborative Care (CC) Case Manager (3CM) works within a Local County Office in the Department of Child Services (DCS) providing regional coverage. The purpose of this position is to assist older youth in transitioning out of the foster care system by being well versed in trauma informed care, engaging older youth using authentic youth-adult partnerships and accessing local resources that benefit older youth transitioning from foster care. It is expected that this position will involve a large amount of travel. The 3CMs duties remain the same as traditional FCMs, however the 3CM will only have youth that are sixteen (16) and older, with a permanency plan of APPLA and/or youth that are over the age 18 and actively participating in the Collaborative Care Program. The 3CM is regionally based and reports to a Collaborative Care Case Manager Supervisor.

What You'll Do:

The essential functions of this role are as follows:

  • Attends Child and Family Team Meetings (CFTM) to give information regarding the Collaborative Care program to youths age 16+ and participates in Regional Permanency Team and Permanency Roundtable as requested as an older youth expert.
  • Visits older youth in out-of-home placements to work with the youth and their team in assessing placement needs to coordinate independent living efforts.
  • Accepts on-call responsibility periodically to assess placement issues, safety concerns, and/or youth who may wish to return to care.
  • Prepares progress, placement review, and permanency reports to the courts, maintains case files and develops briefing reports for in-office case staffings or external provider staffings.
  • Testifies in either a Juvenile or Criminal Court of Law concerning the needs of the youth that are assigned to the incumbent’s caseload.
  • Coordinates with FCMs for any siblings of the older youth and/or children of the older youth if that child is a ward regarding:
    • Sibling visits
    • Scheduling of court hearings
    • Filing of court reports
  • Coordinates with Older Youth Services Providers (if applicable)
    • Making referrals/renewing referrals
    • Monitor services provided to ensure services are consistent with the DCS Older Youth Services Service Standards, best practices and needs of youths
    • Processing KidTraks claims
  • Coordinates local service delivery with contracted and community providers and monitors treatment plans to work towards assisting youth achieve a successful transition to adulthood.
  • Holds regular CFTMs to discuss the strengths and needs of the older youth and to coordinate efforts towards independent living with the youth, providers, placements resources, and other stakeholders.
  • Develops case plans and successful adulthood transitions plans with the older youth to assist them in becoming more self-sufficient and reach their goals to becoming independent. This plan must be developed in a manner that recognizes the unique needs and diverse backgrounds of the youth we work with. Revises plans as needed to meet the dynamic needs and ever changing goals of the youth that we serve.
  • Responds to queries regarding reentry into foster care. Determines eligibility for the CC program and completes VCCA’s with youth when appropriate
  • Maintains data which provides management an opportunity to evaluate the needs of older youth on a trend analysis basis.

What You'll Need to Succeed:
  • Bachelor’s degree from an accredited college/university is required with at least 15 semester hours or 21 quarter hours in Child Development, Criminology, Criminal Justice, Education, Healthcare, Home Economics, Psychology, Guidance and Counseling, Social Work, or Sociology required.
    • BSW or MSW degree preferred.
  • Advanced knowledge of principles and practices of social work, casework, sociology and group and individual counseling including new methods and theories of treatment.
  • Thorough knowledge of state and/or federal laws, regulations, guidelines and standards affecting child protection services, foster care, family preservation and adoption.
  • Thorough knowledge of related social services programs and ability to coordinate work among programs. Must be able to bring service providers to consensus in order to provide services and make recommendations to the court.
  • Specialized knowledge of the interrelationships of the various behavioral sciences as they affect care and treatment of patients/clients and/or objectives of social work projects.
  • Ability to investigate and interview alleged victims and perpetrators of child abuse or neglect in an appropriate and effective manner.
  • Ability to work with families of diverse cultural/ethnic backgrounds with sensitivity to differences in child rearing practices, relationships and needs.
  • Ability to work with children with special needs such as mental or emotional conditions, the foster families or the home willing to accept those children and the professionals who can appropriately provide services to them.
  • Ability to exercise sound judgment while managing crises situations including those involving confrontational, aggressive adults being investigated for child abuse or neglect.
  • Ability to manage multiple priorities, many of a life threatening nature to ensure safety of families and children.
  • Required to posses and maintain a valid drivers’ license, independent reliable automobile and appropriate automobile insurance coverage at all times.   

Training Requirements:  This position requires the FCM to complete a mandatory twelve (12) week training session.  The training session will consist of classroom based modules and transfer of learning modules. Classroom instruction will occur in Indianapolis, Indiana and transfer of learning will occur in the local county office. Family Case Managers must complete minimally 24 hours of training per calendar year.
