Salary Information:

This position traditionally starts at an annual salary of $47,476.  Salary for this position may be commensurate with education and job experience.

About the Job:

In this role, you will serve as a Nurse Surveyor for health care entities licensed/certified or required by law to be inspected by ISDH to determine compliance with state and federal laws and regulations. All surveyors conduct home health and hospice agency surveys.  Experienced surveyors (over 1 year surveyor experience who have federal training) conduct surveys for rural health clinics and end stage renal disease clinics and may be responsible for Quality Assurance, and/or OASIS coordination.  This position is being recruited in Allen County, and qualified individuals from Allen and adjacent counties are encouraged to apply.

A Day in the Life:

In this role, you will:

  • Conduct annual surveys of assigned entities to determine compliance with state and/or federal rules and/or regulations.  This may include recertification surveys, initial surveys, follow-up surveys, complaint surveys, and other applicable surveys within prescribed time frames.
  • Conduct surveys following protocols as defined by SOM, law, policies and procedures of the Centers for Medicare and Medicaid Services, State Agency and Division.
  • Assist in training of division staff and new surveyors.
  • Demonstrate professionalism as you investigate complaints following federal and/or state protocol, policies and procedures.
  • Serve as Team Leader for surveys when assigned.  This includes assigning survey tasks, conducting entrance and exit conferences, and merging and submitting reports timely.
  • Conduct assigned areas of survey when acting under another Team Leader and submitting reports in accordance with Principles of Documentation and QA policy.
  • Perform QA duties when assigned according to Principles of Documentation and QA Policy.
What We're Looking For:
  • A current Indiana Registered Nurse license.
What You'll Need for Success:
  • Specialized knowledge of the nursing principles and techniques being surveyed, as applied to the population being served, is a must.
  • Additional specialized knowledge, including the operation and administration of the type of facility being surveyed will lead to success.
  • You must be committed to the public health mission of the agency and commission of protecting the health, safety, rights, and privacy of patients, clients and residents of the entities being surveyed. 
  • You will have a working knowledge of survey and investigative procedures and the ability to conduct surveys, inspections, interviews, and investigations, and to prepare reports by documenting findings.
  • Practical knowledge of appropriate state and federal laws, including the rules and regulations for licensing and certification of the type of entity being surveyed. 
  • You will have the ability to analyze entity records and documents for compliance with rules/regulations, as well as observe and recognize potential adverse health and safety conditions or rule/regulation violations.
  • You are able to communicate verbally and in writing with individuals and in group situations.
  • Time management and the ability to work independently with little to no supervision is a must.
  • You have the ability to deal with difficult situations and individuals to promote open communication, remaining flexible and maintaining composure under stressful situations.
Supervisory Responsibilities/Direct Reports:
This role may serve as a team lead for an assigned work group.
